Please complete this form and submit to begin the process to terminate your membership. You must provide enough information for the YMCA to locate your account. Per our membership policy, a membership account may only be cancelled for the upcoming month if requested by the 20th of the month. Any requests made after the 20th will be applied to the following month.
NOTE: Your membership has not been terminated until you receive a confirmation email. If you do not receive a confirmation email within one week of your request, please contact the YMCA at (860) 399-9622.
