Please complete this form and submit to begin the process to terminate your membership. You must provide enough information for the YMCA to locate your account. Per our membership policy, a membership account may only be cancelled for the upcoming month if requested by the 20th of the month. Any requests made after the 20th will be applied to the following month.
NOTE: Your membership has not been terminated until you receive a confirmation email during the week of the 20th. If you do not receive a confirmation email within one week of your request, please contact the YMCA at (860) 399-9622 x108.
